Summer To-Do List: Immunizations and Physicals

Immunization Requirements Due by Wednesday, August 13, the first day of the 2025-2026 school year: 

  • All new students must submit a complete up-to-date immunization record.
  • All 7th graders must submit a updated immunization record that includes an updated Tdap Booster and Meningococcal Vaccine (usually given at age 11 or 12).
  • Immunization exemptions are valid for two years. Please ensure your child’s exemption is up to date.

Physical Evaluations: Below are the physical evaluation requirements by grade/due date.

  • All 6th graders and new-to-BCS 7th and 8th graders (even those not participating in athletics) – forms due by the first day of school, Wednesday, Aug. 13.
  • All 6th-8th graders participating in athletics – forms due the first day of tryouts/practice or the first day of school, whichever is first.
  • All 9th-12th graders participating in a high school PE class (Health & Fitness, Strengthening & Conditioning) – forms due by the first day of school, Aug. 13.
  • All 9th-12th graders participating in athletics – forms due the first day of tryouts/practice. Fall athletics begin on Monday, Aug. 4.
  • All marching band students – forms due by the first day of tryouts/practice.

Click HERE for the TAPPS physical form. All three pages must be completed. Please email your student’s completed physical form to Nurse Schmidt at lschmidt@bcsfw.org.

No student will be allowed to try out for/participate in a sport or in marching band unless a current physical is on file in the nurse’s office.
